Qualified Mental Health Professional – Eligible/Trainee

DESCRIPTION:

This position involves the provision of an array of psychoeducational services within the home and community settings as determined by the client’s treatment plan and under the direct supervision of the Clinical Staff Supervisor. QMHP-E’s are responsible for all client related documentation and reports as well as the development and implementation of treatment goals and objectives under direct supervision of the Clinical Staff Supervisor. QMHP-E’s are required to provide their own transportation. On a monthly basis, 90% of the services provided are to be in the form of direct involvement with client and family members. The remaining time will be utilized for service coordination, administrative duties, and trainings. Incumbent must have and maintain a mobile telephone, a valid Driver’s license, automobile insurance, and be available to work evenings and weekends.

 

TASKS:​

  1. Conduct 8-10 hours a week per client as identified in assessment.

  2. Participate in the initial thirty-day assessment of client/family situations and ongoing monthly reports following the approved format. Provide a service plan within the initial thirty-day period, revising as needed there after to reflect client progress and confirmed needs.

  3. Monitor length of client services and notify referral sources prior to the completion of ninety- day period to determine if continuation is necessary.

  4. Have weekly contact with the Referring Agency.

  5. Arrange for weekly case supervision with clinical supervisor.

  6. Act as a co-leader of parenting and peer education/support groups as needed.

  7. Act as the liaison between family, referring agencies, and other community organizations.

  8. Coordinate client services between various agencies.

  9. Maintain case records and document services rendered.

  10. Attend weekly staff meetings consistently. Prior approval from management team is required when absent.

  11. Prior to the end of each quarter service period, coordinate and facilitate a staffing for the client, inviting the family of the client, the Referral Source, representation from other pertinent agencies providing services and supervision.

  12. Maintain high degree of client confidentiality.

  13. Work with other staff assigned to cases, communicating effectively.

  14. Maintain a high degree of professionalism in the community, with clients, and with referring agencies seeking to build and maintain positive relationships.

15. Provide Individual and Family counseling services to clients in the home, school and community setting.

16. Provide Crisis Intervention as needed.

MINIMUM QUALIFICATIONS:

 

Candidate must have a Bachelor Degree in an approved Human Services and or Related Field. Candidate must be registered or be prepared to register as a Trainee with the Department of Health Professions Board of Counseling or Board of Social Work. Candidate must have a mobile phone and reliable transportation.

The Board of counseling may consider other degrees in human services or in fields related to the provision of mental health services.
