Qualified Mental Health Professional- Adult (QMHP-A)
DESCRIPTION:
This position involves the provision of an array of case management, training, support, crisis intervention, coordination of care, and activities of daily living services to adults with serious mental health diagnoses, emotional, and behavioral challenges. QMHP-A’s are responsible for all client related documentation and reports as well as the development and implementation of treatment goals and objectives. The individual serviced through this program will require supports with preventing hospitalization, balancing the demands of daily living skills and regular maintenance required to keep their mental, emotional and behavioral challenges stabilized. The QMHP-A will be required to meet the individuals several times throughout the week to support them towards advancement with the goals that have been outlined in their Individualized Treatment plans. These sessions will require creative, hands-on trainings and applications all in an effort to assist each individual towards improvements in their daily living and mental / emotional state. QMHP’s will need to identify resources within the community to support the individual’s ability to continue to live and thrive within their homes and communities without the threat of homelessness, hospitalization, incarceration or any other maladaptive conditions.
Incumbent must have and maintain a mobile telephone, a valid Driver’s license, automobile insurance, and be available to work evenings and weekends.
TASKS:

Participates in initial assessment of client/family situations and ongoing monthly reports following the
approved format.

Participates in the service plan within the initial thirty-day period.

Monitor length of client services and notify referral sources prior to the completion of ninety-day period to
determine if continuation is necessary.

Have weekly contact with the Referring Agency.

Arrange for weekly case supervision with clinical supervisor.
-
Act as a co-leader of parenting and peer support groups as needed.

Act as the liaison between family, referring agencies, and other community organizations.

Coordinate client services between various agencies.
-
Maintain case records and document services rendered.

Attend weekly staff meetings consistently. Prior approval from management team is required when absent.

Prior to the end of each Ninety-day service period, coordinate and facilitate a staffing for the client, inviting
the family of the client, the Referral Source, representation from other pertinent agencies providing services
and supervision.

Maintain high degree of client confidentiality.

Work with other staff assigned to cases, communicating effectively.

Maintain a high degree of professionalism in the community, with clients, and with referring agencies
seeking to build and maintain positive relationships.

Assist in providing Individual and Family counseling services to clients in the home and community setting.

Provide Crisis Intervention as needed.

Provide other duties as assigned.

Minimum Qualifications:
Candidate, by education and experience is professionally qualified to provide mental health services to adults and is registered with the Board of Counseling as a QMHP-A.

The Board of counseling may consider other degrees in human services or in fields related to the provision of mental health services.
